PRACTICE HOURS: Office
hours are 8:00 a.m. to 5:00 p.m., Monday through Friday. Please call our office to schedule your appointment. (Toll free in NH: 1-800-742-5800 or 603-224-1223) We will do our best to accommodate your scheduling needs.
BILLING OFFICE HOURS:
Our billing office is open from 8:00 a.m. to 4:45 p.m. Monday through Friday. Our Patient Account Representatives are available to assist you with any questions regarding your insurance claim or self-pay balance and can be reached at 228-7261.
LOCATION: We are conveniently located in the Memorial Building, Suite 106, on the Concord Hospital Campus.
WHAT TO BRING TO YOUR APPOINTMENT:
- Current insurance information and cards
- Co-payment (If applicable)
- You may be asked to bring x-rays and medical records.
CHILDREN LESS THAN 18 YEARS OF AGE must be accompanied by a parent or legal guardian. If the parent or guardian is unavailable, a letter giving authorization is required.
TELEPHONE CALLS AND PRESCRIPTION REFILLS: Our telephones are answered from 8:30 to 12:00 and 1:00 to 5:00. Our patients are encouraged to call with any medical questions or concerns.
We will be happy to take a message for the doctors. We ask for 24-hour notice for prescription refills. Prescriptions will not be refilled after office hours, except on an emergency basis.
AFTER HOUR CALLS will be answered by
our answering service. Emergency calls will be forwarded to the physician on call and returned at the earliest opportunity.
MEDICAL RECORDS: In order for medical records to be released we ask for a written request signed by the patient or guardian giving Quality Orthopaedic Care permission to release the records.
INSURANCE:
Our office participates with most major insurance carriers and will submit your bills directly to them. If your insurance requires a referral, please be sure to contact your primary care physician.
WORKERS’ COMPENSATION:
Please provide us with your employer’s full name, telephone number and mailing address as well as your claim number and date of injury.
