Every patient is important to us at Quality Orthopaedic Care, P.C.
We want to make the most of your time and provide you with a comprehensive and thorough evaluation. We will take the time to answer your questions and discuss your treatment options.

If this is your first appointment with us, please bring the following items:

• Completed New Patient Packet -

• A list of your current medications (dosage and frequency)
• A list of questions and concerns
• Any recent x-rays of the problem area – x-rays taken at Concord Hospital or Concord Imaging can be viewed electronically through our secure systems
• Any records you can acquire relating to the problem
• Your health insurance card and co-pay (if applicable) - We are a specialist office which, with some insurances, requires the higher co-pay
• If needed, a friend or family member to take notes or help you to the appointment - (Patients under 18 years of age DO need to have a parent or guardian with them for the appointment)

• Our office is located on first floor of the Memorial Building, nearest the Hospital end of building.
