Frequently Asked Questions1.) Office hours: Monday through Friday 8:00am to 5:00 pm.
hide this answer We are best suited to assist you during regular office hours. However if problem arises during non-office hours, call our office number and our answering service will take your information and forward it on to the on-call physician, who will return your call.
hide this answer Prescription refills are best handled during regular office hours and should be called at that time. Please be aware of amounts of your medications. Prescription refills require a 24hour notification to be processed.
hide this answer Medical questions are taken as messages and given to the providers. Please be clear and concise and provide the best method to contact you. The providers answer the questions as they are able throughout the day. Some may not be returned until the end of day.

hide this answer To get a copy of your medical records, you must fill out a medical record release form and return it to our office. Processing of your request may take up to one week.

You will be required to furnish our office with a copy of your insurance card and be responsible for any co-payments or deductibles as required by your insurance. Please be aware of limitations and/or exclusions regarding your policy. You may call our office if you have specific concerns regarding your insurance.

Most insurance cards inform you on the face if you need a referral, HMO’s generally require referrals. If you are unsure you may contact your insurance carrier, your human resource department, or primary care provider. Our office cannot provide you with your referral information.
hide this answer 12.) Re-visit items: If you have not been in the office for awhile, you will be asked to fill out some new paperwork. This is to ensure that we have your updated medical history, medication and insurance information. Often small changes in this kind of information can make big changes. We appreciate your patience.

Cancellations: In consideration for other patients waiting for appointments we request that if you cannot make an appointment, that you cancel at least with 24 hours notice. This allows us to accommodate patients on a waiting list.
hide this answer 14.) "No-showing" for an appointment: "No-showing" for an appointment means that other patients that could have been helped are not. Please contact us if you wish to be re-scheduled.
